ACKNOWLEDGEMENT OF ACTIVITY I, the undersigned participant (“Participant”), understand that PrecisionSim provides interactive simulation shooting and training experiences using non-live-fire equipment. Activities may include: Simulation firearms, laser-based or CO₂-powered training weapons Recoil-enabled training systems Projected video and moving targets Physical movement (walking, turning, aiming) Loud sounds and flashing lights in realistic scenarios I understand that NO LIVE FIREARMS OR LIVE AMMUNITION ARE PERMITTED at PrecisionSim. 2. ASSUMPTION OF RISK I understand and voluntarily assume all risks of participation, including but not limited to: Slips, trips, or falls Strains, sprains, bruises, or minor injuries Equipment malfunctions Eye or hearing discomfort Emotional stress or anxiety from realistic scenarios Aggravation of pre-existing medical conditions I voluntarily accept all risks, whether caused by my actions, the actions of others, equipment, or the facility. 3. SAFETY RULES I agree to follow all staff instructions and posted safety rules, including: Treat all training weapons as if they were real Keep muzzle pointed in a safe direction Keep finger off the trigger until ready to engage the simulation No horseplay or unsafe conduct Use protective equipment as instructed I understand PrecisionSim may remove me from the facility without refund for unsafe behavior or refusal to follow rules. 4. NO LIVE WEAPONS / AMMUNITION I acknowledge and agree: No live firearms are allowed on the premises No live ammunition is allowed Any violation may result in denial of entry or removal 5. RELEASE OF LIABILITY In exchange for being allowed to participate, I release, waive, and discharge PrecisionSim, its owners, employees, instructors, volunteers, agents, and affiliates (“Released Parties”) from any claims or liability for injury, loss, property damage, or death arising from participation, including claims arising from negligence, to the fullest extent permitted by law. 6. INDEMNIFICATION I agree to defend, indemnify, and hold harmless the Released Parties from any claims, lawsuits, damages, or expenses caused by my actions, negligence, or failure to follow rules. 7. PROPERTY DAMAGE I understand I may be charged for damage caused by misuse, negligence, or intentional actions to equipment or facility property. 8. MEDICAL ACKNOWLEDGEMENT I confirm that I am physically and mentally capable of participating. I will stop immediately if I feel unsafe or unwell. Participants with pre-existing medical conditions or pregnancy participate at their own risk. 9. PHOTO / VIDEO RELEASE (OPTIONAL) I grant PrecisionSim permission to photograph and/or record me for marketing purposes. 10. GOVERNING LAW This waiver is governed by the laws of the State of Maine. If any part is unenforceable, the remainder remains in full force. 11. ACKNOWLEDGEMENT & SIGNATURE By signing this waiver, I confirm that: I have read and understand this agreement I voluntarily accept all risks and responsibilities I release PrecisionSim and associated parties from liability 12. PARENT/GUARDIAN CONSENT (UNDER 18) I am the parent or legal guardian of the minor named above. I consent to the minor’s participation and agree to all terms of this waiver.
